Have you been admitted to hospital because of mental health problems and require help when discharged back home?

You may be eligible for free aftercare if you have been in hospital under certain sections of the Mental Health Act 1983. You can get it if you have been:

  • detained in hospital for treatment (section 3)
  • transferred from prison to hospital (sections 47 or 48)
  • ordered to go to hospital by a court (sections 37 or 45A).

The Discharge Process

  1. Admission into hospital under s3, s37, s45A ,s47 or s48 (You will not get free aftercare if you have been in hospital under section 2, 4, 5 or 38 of the Mental Health Act 1983)
  2. Hospital Discharge Assessment - looks at needs you're likely to have once discharged or transferred from hospital
  3. NHS or Social Services will prepare care plan & package depending on your needs
  4. Regular reviews
  5. Reassessment of needs if necessary - i.e. change of circumstances
